How can I make an appointment and what is the cancellation policy?

We ask that you call in advance so that a doctor’s appointment may be reserved for you. However, we will accommodate situations that require same-day care. We respect your time and make every effort to remain on schedule, and ask that you extend the same courtesy to us. If you are unable to keep an appointment, please notify the appropriate primary care office immediately.

 

What if I need to see a doctor while the office is closed?

We recognize that urgent medical situations do arise, and we will respond to you as quickly as possible. If an urgent need arises while the office is closed, our answering service will promptly forward your message to the doctor who is on call. You can also visit Immediate Care Medical Walk-In in the evenings and weekends when our offices are closed.

If you feel your medical condition is an emergency, please call 911 immediately.

 

May I call my doctor with questions or concerns?

We encourage you to call with questions you may have concerning your treatment. If your call requires that the doctor speak with you, please leave a message and the doctor will return your call at the earliest opportunity.

 

What are the policies for fees and payments?

We make every effort to keep the costs down while maintaining a high level of professional care. You can help by paying for your treatment at the time of your visit. There will be a $2 service charge for co-payments not paid at the time of service.

 

Does Family Practice of CentraState accept my insurance?

Our office accepts Medicare and most managed care plans. Insurance is intended to cover some, but not all of the cost of your care. Most plans include co-insurance, a deductible and other expenses, which must be paid by the patient.
Please bring your insurance plan information with you on your first visit. We will work with you to assure that you receive the maximum benefits to which you are entitled; however, please always contact your insurance company before your visit for coverage details.

 

How can I request a refill for my prescription(s)?

Please remember that all prescriptions and authorizations for renewals should be requested during normal office hours. No prescriptions will be filled after normal business hours and/or on weekends. Please plan accordingly. Our practice can send prescriptions electronically to pharmacies. This means a safer, more efficient prescribing process for you and your family. The CentraState Pharmacy is conveniently located in CentraState Medical Center’s Star and Barry Tobias Ambulatory Campus for our Freehold office patients.

 

What is the smoking policy?

Because exposure to tobacco is hazardous to our health and yours, we ask that you refrain from smoking while in our office.

 

May I use my cell phone while in Family Practice of CentraState?
