We understand that situations arise in which you must cancel your appointment. It is therefore requested that if you must cancel your appointment you provide more than 24 hours’ notice. This will enable another patient who is waiting for an appointment to be scheduled in that appointment slot. With cancellations made less than 24 hour notice, we are unable to offer that slot to other patients. Office appointments which are cancelled with less than 24 hours notification may be subject to a $25.00 cancellation fee. The Cancellation fees is the sole responsibility of the patient and must be paid in full before your next appointment. We understand that special unavoidable circumstances may cause you to cancel within 24 hours. Fees in this instance may be waived but only with management approval. Questions about cancellation fees should be directed to the Billing Department (732-462-6373).
Patients who do not show up for their appointment without a call to cancel an office appointment will be considered as a “NO SHOW.” Patients, who are a no-show three or more times may be dismissed from the practice and will be denied any future appointments. Patients may also be subject to a $25.00 fee for an office appointment no show. The no-show fee is the sole responsibility of the patient and must be paid in full before your next appointment. We understand that special unavoidable circumstances may cause you to miss an appointment. Fees in this instance may be waived but only with management approval. If an urgent need arises while the office is closed, our answering service will promptly forward your message to the doctor who is on call. You can also visit Immediate Care Medical Walk-In in the evenings and weekends when our offices are closed. If you feel your medical condition is an emergency, please call 911 immediately.
We encourage you to call the office with questions you may have concerning your treatment. If your call requires that the doctor speak with you, you will be asked to leave a message and the doctor will return your call at the earliest opportunity.
Our office accepts Medicare and most managed care plans. Insurance is intended to cover some, but not all of the cost of your care. Most plans include co-insurance, a deductible and other expenses, which must be paid by the patient. Please bring your insurance plan information with you on your first visit. We will work with you to assure that you receive the maximum benefits to which you are entitled; however, please always contact your insurance company before your visit for coverage details.
Please remember that all prescriptions and authorizations for renewals should be requested during normal office hour. Some prescription refill requests may require a physician visit. No prescriptions will be filled after normal business hours and/or on weekends so please plan accordingly. Our practice sends prescriptions electronically to pharmacies. This means a safer, more efficient prescribing process for you and your family. The CentraState Pharmacy is conveniently located in CentraState Medical Center’s Star and Barry Tobias Ambulatory Campus for our Freehold office patients. And there is a pharmacy on site at the Marlboro Medical Arts Building for our Marlboro patients.
